Crawford County Public Health holds May & June Clinics 2021

                                       By Appointment Only and Mask Are Required.       

Flu Shots are still available. Call office for details.  It’s not too late to protect you, your family, friends,

co-workers or those you come in contact with.  It’s important that everyone gets immunized.

           

Baby Boutique will be held by appointment only on May 24th & June 28th, 2021 at the United Methodist Church in Denison. Please call 712-263-3303 for an appointment.   Store hours are 3:00 to 6:00 p.m. 

Baby Boutique is a service to promote healthy children and wellness through education and medical care for prenatal and children up to the age of one with an opportunity to earn points to purchase baby items.                                                                                                                                                            

                                                                                                   

Immunizations Clinics: Clinics will be held Monday May 10th, 2021 from 1-5:30 pm, & Thursday May 20th 2021 from 10:00 a.m. 2:00 p.m.  Immunization Clinics shots, by appt. only (mask required) will be Monday June 7th, 2021 from 1-5:30 pm & Thursday 17th, 2021 from 10:00 am -2:00 pm.

  It is very important to be on time for your scheduled appointment. If you are unable to attend the scheduled date, please call and cancel as soon as possible.  At Immunization Clinic, nurses give recommended immunizations to children 2 months old through 18 years of age. Recommended Immunization schedules are 2, 4, 6, 12, 15, & 18 months & 4 years of age.  Children who are 11-12 years of age need to be immunized with Tdap Booster, Meningitis & HPV. Children can be immunized for DTaP, MMR, Hib, Hep B, IPV, Varicella (chicken pox) Prevnar, Tdap, Meningitis, Hep A, Rotavirus, & HPV.

Parents must accompany the child to the clinic.  A fee of $10 per child is charged to help defray costs, excluding Medicaid clients.  Please bring child’s social security number, all immunization records and Title 19 card (if applicable).   If your insurance covers immunizations, including hawk-i insurance, please take your child to your family physician.

                                                                                                                                                                             

Lead Screening:  Available at all Immunization Clinics for children 12 months to 6 years old.

Oral Health Screening: Available at all Immunization Clinics for children.

Hawk-i Health Insurance:  Insurance for children ages 0 to 19 years old, based on yearly income.  Staff member will be available to provide information and help in completing application.

 

Child Health Clinic:  Please call 263-3303 for information.  Child Health provides a variety of medical services based on the child’s age and needs.  Services may include physical examinations, immunizations, vision testing, doctor and dental referrals, nutritional and family counseling.

Maternal Health Clinic:  Please call 263-3303 for appointment or additional information.  Maternal Health Program offers educational information regarding maternal health, as an extension of the information received from the mother’s-to-be physician.  Services include nutritional and family counseling and essential information regarding the entire birth experience, from conception through pregnancy and the actual birth.                                                                                                                                                                                                                                                                                                                                                      

 

Family Planning Clinic:  Please call 263-3303 for appointment or additional information. Family Planning offers birth control, pregnancy tests, physical exams, cancer screenings, pap smears, breast exams, blood pressure, anemia screenings, tests and treatment for sexually transmitted diseases.  Confidential HIV screening and counseling are also available. Proof of U.S. Citizenship, proof of identity and proof of one full month’s income (check stubs for the entire month) is required.

  

 

Iowa Breast & Cervical Cancer Early Detection – Care for Yourself Program:  A special service for women age 40 to 64. You may be eligible to receive a variety of health examinations at no cost to you if you have no health insurance, a policy that does not cover these services, or you cannot pay your deductible or co-insurance.
