RELEASE: St. Anthony Hospice, Home Health, & Public Health to Relocate Offices
Beginning Thursday, July 15th, St. Anthony Home Health, Hospice, and Carroll County Public Health will relocate to their new office space within the St. Anthony Hospital building on the south end of the 2nd level near Iowa Heart Center and St. Anthony Chronic Care Center. They had been located in the Luke Building east of St. Anthony Nursing Home for many years.
St. Anthony Home Health, Hospice, and Carroll County Public Health’s new address will be 311 South Clark St. The phone number will remain the same: (712) 794-5408. Office hours continue to be Monday through Friday, 8:00 am to 4:30 pm.
